Abstract: Occlusal trauma represents one of the local contributing factors to periodontitis. Standardized intervention in occlusal trauma holds significant importance in comprehensive periodontal therapy. This article reports the multidisciplinary management of a case of severe periodontitis with occlusal trauma affecting bilateral maxillary first premolars. Treatment procedures included basic periodontal therapy，occlusal intervention，periodontal regenerative surgery，alveolar ridge preservation and implant therapy，and through combination with standardized supportive periodontal care，the following results were achieved: effective control of periodontal inflammation，establishment of stable occlusal relationships，and restoration of masticatory function. Furthermore，we discuss the similarities and differences in the diagnosis and treatment strategies for the bilateral affected teeth in this case，aiming to provide clinical experience for similar cases.

摘要： 咬合创伤是牙周炎的局部促进因素之一，其规范干预在牙周系统治疗中具有重要意义。文章报告1例双侧上颌第一前磨牙重度牙周炎伴咬合创伤患者的多学科诊疗过程，其通过牙周基础治疗、咬合干预、牙周再生性手术、拔牙位点保存、种植修复，并结合规范的牙周支持照护，实现了牙周炎症的有效控制、稳定咬合关系的建立及咀嚼功能的恢复；同时，讨论该病例中双侧患牙诊治思路的异同，以期为此类病例提供临床经验。
