Abstract    Locally recurrent gastric carcinoma is commonly referred to the gastric carcinoma arising from the operating field or the gastric remnant. Because most locally recurrent gastric carcinomas arise from the operating field, it is difficult to obtain a histological diagnosis. Curative resection of the primary tumor and adjacent tissues leads to formation of a wide range of adhesions surrounding the operating field, including a number of important organs and blood vessels. Local recurrence of the carcinoma is tended to involve the surrounding important organs or blood vessels, in most cases where complete resection is not possible. Local recurrence of the carcinoma involving the adjacent organs usually invades the lymphatic vessels of the corresponding organs, so that the lymph node metastasis range is more extensive. Surgical resection is still the first choice for locally recurrent gastric carcinoma, but a small number can be resected. Accurate preoperative diagnosis is the first step to prevent local recurrence. Besides, standard operation and necessary preoperative chemotherapy are also important steps.

摘要：

通常将局部手术野以及残胃上的复发癌称为局部复发性胃癌。因多数局部复发性胃癌来源于局部手术野，难以获得组织学诊断。胃癌根治术对原发灶及其邻近的组织进行了广泛的切除后，就形成了以手术野为中心的、包含诸多重要脏器、血管在内的广泛粘连。癌组织局部复发后就非常容易累及粘连于周围的重要脏器或血管，多无法切除。局部复发癌在累及邻近脏器的同时也侵入相应脏器的淋巴网络，使其淋巴结转移范围更广泛。手术切除仍是局部复发性胃癌的首选方法，但能够切除者不多。初次手术时准确的术前诊断是防止局部复发的第一环节，规范性手术、必要的术前化疗则是预防复发的重要步骤。
