Abstract    Infertility is a rare complication after inguinal hernia repair. Male inguinal hernia has a close anatomical relationship to the spermatic cord. Injury to spermatic vessels may cause postoperative ischemic orchitis，testicular atrophy，and endocrine dysfunction. Intraoperative injury to vas deferens or postoperative pathological changes at the surgical site may cause vasal obstruction resulting in oligospermia or azoospermia. Both vascular and vas deferens injury may lead to infertility. Therefore，various types of inguinal hernia surgery have always emphasized the operative protection of spermatic cord integrity. In addition，testicular ischemia，vas deferens injury or obstruction may also cause damage to the blood-testis barrier，followed by production of anti-sperm antibodies which may lead to immune infertility that requires attention from surgeons.

摘要： 不育、不孕及性功能障碍等生殖功能损伤是腹股沟疝修补术后的一种罕见并发症。男性腹股沟疝与精索解剖关系密切，精索血管损伤可以导致缺血性睾丸炎、睾丸萎缩以及内分泌功能障碍，输精管损伤可以导致梗阻性无精症，均可能引起不育。因此，各类腹股沟疝手术对于精索完整性的保护操作一直是强调的重点。此外，睾丸缺血、输精管损伤或梗阻还可能造成血睾屏障破坏，使机体产生抗精子抗体而导致免疫性不育，这也需要外科医生予以重视。
