循肝静脉途径肝切除相关精细解剖学基础和要点

摘要: 肝静脉系统是肝脏分叶、分段的核心解剖标志。循肝静脉途径是提高解剖性肝切除精准性和疗效的关键环节之一。循肝静脉途径肝切除所涉及的精细解剖学基础,包括肝左、肝中、肝右静脉及其属支的走行及变异;实施循肝静脉途径肝切除的关键技术要点包括肝静脉的入路及解剖、显露技巧、段间静脉的识别与保护、肝静脉流域与门静脉流域差异的应对等。随着高分辨率多模态导航技术的发展,对门静脉流域及流域间静脉的认知已进入亚肝段水平,将进一步提升肝切除术的精准性。

Abstract: The hepatic venous system serves as the key anatomical landmark for the division and segmentation of the liver. The approach along the hepatic veins is one of the key steps to improve the accuracy and efficacy of anatomical liver resection. The detailed anatomical basis involved in liver resection via the hepatic vein approach includes the course and variations of the left, middle and right hepatic veins and their branches; the key technical points for performing liver resection via the hepatic vein approach include the approach and anatomy of the hepatic veins, exposure techniques, identification and protection of inter-segmental veins, and response to the differences between the hepatic vein drainage area and the portal vein drainage area. With the development of high-resolution multimodal navigation technology, the understanding of the portal vein drainage area and the veins between drainage areas has reached the sub-liver segment level, which will further enhance the accuracy of liver resection surgery.
