    Thinking of treatment for orthodontic borderline cases

    Abstract:In orthodontic treatment,we may often deal with borderline cases,in which both extraction and non-extraction treatment may be suitable,or both surgery and non-surgery treatment may be reasonable. Thus,the treatment plan for borderline cases troubles orthodontists. Based on principles and essence,this article discussed the factors influencing extraction decision-making and the qualitative and quantitative indications of orthognathic surgery,and analyzed the challenges and traps in dilemmas.

    Abstract:Application of implants in restoration of edentulous will significantly improve the treatment outcome as well as the patients’ satisfaction. For maxillary and mandible edentulous,the author introduced the locating of implant sites as well as the amount of implant,and the design of the abutment,based on the evidence from clinical trials and the basic researches. The objectives of this article were to present the guidance for the clinicians to make treatment option for edentulous patients.

    Abstract:Fixed implant-supported prosthesis for the edentulous maxilla has gained tremendous popularity over these years. However,fixed prosthetic rehabilitation of the edentulous maxilla is known to be challenging and requires meticulous planning. This is mainly due to the natural anatomy of the maxilla,the pattern of bone resorption,quality of bone for implant placement,development of prosthetic emergence profile,oral hygiene issues,role of the teeth and hard tissue in speech,and the importance of the prosthesis on facial and dental esthetics. This article reviews pertinent dental and facial esthetics literature for application of various esthetic concepts involved in diagnosis and treatment planning for an implant-supported fixed prosthesis in the edentulous maxilla. Analysis of esthetic parameters in superior-inferior analysis can help the clinician lay the foundation for esthetic diagnostics of maxillary implant-supported fixed prostheses.

    Abstract:CAD/CAM technology has been applied to dentistry for over 20 years. Its rapid development and improvement have broadened the scope for prosthetic implant treatment and brought profound changes to it. Traditionally, mucosa-supported f dentures for patients with edentulous jaws cannot acquire reliable retention and restore efficient mastication,which is due to poor alveolar bone conditions. However,implant-supported fixed dentures radically overcome the problem of retention and stabilization,but implant-supported long fixed bridges cannot be fabricated and placed with ease. Prostheses made by computer-aided design and computer-assisted manufacturing (CAD/CAM) possess fit accuracy,ideal mechanical properties and biocompatibility. The present article introduced the CAD/CAM technology on its beginning,development as well as its application (constitution,prosthetic process and precautions for use) by describing a case of a patient with edentulous jaws and severely atrophic alveolar bone who had received the implant supported fixed restoration by CAD/CAM in order to provide indications for clinical applications

    Abstract:Implant-supported dental prosthesis is documented with high and predictable implant survival rate for edentulous patients. The pattern of implant rehabilitation has been improved for decades. The progress is involved in implant immediate loading, changing of implant superstructure, and digital technique using in implant prosthesis. The aim of the review was to summarize the change of the implant superstructure and improvement of the implant treatment design in completely edentulous jaws in order to get some useful suggestion.
    Experimental research of PAN/CS/BCP/teasel composite scaffold material in repairing alveolar bone defect

    Abstract:Objective To study the role of PAN/CS/BCP/teasel composite scaffold material in bone healing and repair of alveolar bone defect. Methods Totally 24 rabbits were made into alveolar bone defect models with bilateral diameter of 10 mm, and they were divided into two groups: PAN/CS/BCP/teasel group (experiment group) and control group (implanted with chitosan/nano hydroxyapatite). Respectively at the end of 4, 8 and 12 weeks, two rabbits in each group were put to death in order to make bone density analysis and histological observation. Results (1) In experiment group, the bone defect edge showed high density resistance photograph and its density was the same as normal bone tissue. (2) For each period, the new bone formation rate in the experiment group was higher that in the control group. (3) Histological observation: HE dyeing showed ossification line and relatively regular bone formation.Conclusion PAN/CS/BCP/teasel composte scaffold material can hasten bone regeneration and promote bone healing.
    Application of computer-aided design plus homemade tooth support guides in to the implantation the area of the posterior mandible

    Abstract:Objective To evaluate the value of computer navigation +homemade surgical guide technique in the area of the posterior mandible. Methods Make model of the teeth lost region,then select implantation poin and the first guide was made. CT data was collected and Simplant software was used for data analysis;use a grinding instrument to determine the direction of the final implantation and final surgical guides. The guides were worn,then measure preoperative and postoperative angle and depth deviation. Results By comparing the measured data,the portion of implants in the buccolingual direction had a deviation,which was about 10 degree;angulation abutment can adjust the dental prosthetic restoration to a correct occlusion relationship. Planting depth and mesiodistal had small deviations before and after the implant. Conclusion The system can help clinicians to implant under inadequate height conditions,which is of great significance in improving the quality and accuracy of implant surgery.
    Preliminary study of osteogenic effects of HAG/PDLSC complex in bone reconstruction of alveolar cleft in rabbits

    Abstract:Objective To evaluate the osteogenic function of hyaluronangel combined with periodontal ligament stem cells on reconstruction of alveolar cleft in rabbit. Methods Alveolar cleft was surgically made in 24 rabbits,They were treated with HAG/PDLSCs in the experimental groups. Single periodontal ligament stem cells and single HAG carrier application served as control groups. Those with nothing added to the bone defects were regarded as blank controls. Histopathologic method,immunohistochemical staining, and histomorphometry were used to evaluate the osteogenic effects in the above groups. Results There was no significant difference between the control group and blank control group. However,significant difference was verified in the HAG/PDLSCs group. Conclusion It is verified that HAG/PDLSCs can promote osteogenesis in rabbit alveolar cleft reconstruction. PDLSC has the positive functions in alveolar osteogenesis.
    The application of computer-controlled local anesthetic delivery system STA therapy in the dental anxiety

    Abstract:Objective To observe the effect of the computer-controlled local anesthetic delivery system STA(Single Tooth Anesthesia)in the treatment of dental anxiety. Methods Totally 100 patients with pulpitis and dental anxiety were randomly divided into experiment group and control group. STA system was performed in the experiment group(n=44) and traditional injection technique was conducted in the control(n=56). The reaction time and expiration time were recorded. Then the patients were asked to evaluate the pain. The efficiency rate was recorded. Results The reaction time and expiration time in experiment group were shorter(P < 0.05). During the anesthesia process,the pain in experiment group was less than those in the control(P < 0.05).The anesthesia efficiency of experiment group was significantly higher. Conclusion STA system can effectively degrade the level of the fear.

    Abstract:Objective To explore the effects of the static magnetic field of magnetic functional appliance on the morphology and ultrastructure of the SD rat’s maxillofacial skeleton muscle cells. Methods Loading the static magnetic field with magnetic field intensity 971.77 mT on the third generation of the SD rat’s maxillofacial skeleton muscle cells,morphological changes were observed under inverted microscope;ultrastructure changes were observed under transmission electron microscope. Results The cells were in normally cell morphology and good adaptability was observed by inverted microscope. The cytoplasmic free ribosomes, Golgi apparatus,glycogen,mitochondria,rough-endoplasmic reticulum and microfilaments were observed by TEM. Conclusion The simulating static magnetic field of magnetic functional appliance has no damage to morphology and internal structure of the cells. Static magnetic fields have the biological effects of promoting cell growth.
    Application of jaw-closing impression technique with silicone rubber materials to fabrication of complete denture

    Abstract:Objective To compare the effect of two ways’ impressions in edentulous jaws prosthodontics:jaw-closing impression and jaw-opening impression. Methods Thirty cases of complete denture patients were chosen, and randomly divided into two groups;complete dentures were made by jaw-closing impression and jaw-opening impression respectively, then the adsorbability and sore spots modifications were checked after three months. Results The adsorbability of complete denture fabricated with jaw-closing impression technique was better and the times of sore spots amendment were less. Conclusion In fabrication of complete denture, jaw-closing impression technique with silicone rubber materials is better than jaw-opening impression technique.
    Study of bracket bonding with flowable composites

    Abstract:Objective To evaluate the adhesive effects of Esthet-X Flow flowable composite resin by analysing the shear bond strength(SBS) and adhesive remnant index(AIR). Methods A total of 30 maxillary premolar teeth were randomly divided into three groups. All the teeth were prepared with 35% phosphoric acid for 30 seconds. In experimental groupⅠ,the teeth were conditioned with prime & bond NT primer,and the brackets were bonded with Esthet-X Flow flowable composite resin. In experimental groupⅡ,the brackets were bonded with Esthet-X Flow flowable composite resin directly. In control group,the brackets were bonded with Jingjin enamel adhesive. All the specimens were stored in distilled water at 37℃ for 24 h. After storage,SBS was measured,and ARI was quantified by using an optical microscope. Results SBS of two experimental groups was lower than and had statistical difference with that of control group(P < 0.05). There was no obvious difference between two experimental groups (P>0. 05). ARI hade no obvious differences among the three groups(P>0.05). Conclusion Using prime & bond NT primer or not has no effect on bond shear strength of Esthet-X Flow flowable composite resin. Esthet-X Flow flowable composite resin can satisfy the clinical need of bonding bracket.

    Abstract:Bruxism is a common disease which draws more and more attention in dentistry,and is considered a risk factor of implant restoration. However,controversies still exist in the relationship between bruxism and dental implant failure,as well as the special managements of bruxism patients for implanting. Bruxism causes overload of dental implants,bringing biological complications and biomechanical complications. Practical guidelines involve treating bruxism,special design of implants’ number,dimensions and occlusal patterns,applying occlusal splints,etc. This article focuses on effects of bruxism and possible managements for implant restoration.

    Abstract:The magnetic attachment is a precision attachment that utilizes magnetic attraction adsorbed onto abutments or implants. Magnetic attachment not only has the characteristics of removable partial dentures,but also has its own advantages. It can significantly improve retention force and chewing efficiency of the removable partial denture,and serve well; however,there are some problems in its use. In this present paper,we review the characteristics and problems in the use of magnetic attachment.
