    Recognization and suggestion on segmented fixed implant supported complete denture

    Abstract:Periodentitis is one of major risk factors for edentulous and induces dramatic jaw bone absorption. When designing segmented fixed implant supported denture we should consider the following key factors:timing and principles of these loose teeth extraction,defects of hard and soft tissue,the diversity of bone absorption in different area,disorders and instability of maxillomandibular occlusion,edentulous before final prosthodontic,retentive patterns of implant supported denture,occlusion designs,preventing bone absorption surrounding implants,maintaining oral health and reducing long-term maintenance costs. This article summarizes and analyzes the patients with segmented fixed implant supported denture in the Implant Center of West China College of Stomatology,discusses the problems and the solutions during the process of restoration design,implant surgery,and immediate and final restoration.

    Abstract:Nowadays,with the development of digital information technology,computer technology has been applied to dental implant field. Treatments for edentulous patients with digital surgery guide not only improve the accuracy of implant placement,but also reduce surgical trauma and achieve the immediate loading. This article will report the protocol of digital treatment planning of implant-supported fixed prostheses in edentulous patients and discuss the advantage,limitation and shortcomings of the computer-assisted surgery guide.
    Analysis of long-term success factors for implant-supported rehabilitation in the edentulous cases

    Abstract:The conventional complete denture with insufficient stability and supportiveness is often difficult to achieve ideal restoration results. Long-term studies have demonstrated that the use of implants can obviously increase the stability and comfort level,and effectively reduce the absorption of alveolar bone,which has a great impact on the prosthodontic treatment for the edentulous patients. In this review,we are going to discuss and analyse the main factors that affect the long-term success for implant-supported rehabilitation in the edentulous cases.

    Abstract:The effect of edentulous mandibles with serious alveolar bone absorption restored with traditional complete denture was always bad because of mandible movement,lower alveolar ridge area,muscle adhesion and the influence of saliva and tongue,thus some other devices were often needed to improve denture retention and stability. The implant-supported overdentures not only increased denture retention and stability,but also provided a support type of implant and mucosa,the absorption of the alveolar ridge slowed down,and applied in clinic more and more widely. This paper researched related articles from OVID and PUBMED databases and compared them with a period of before,and reported a review about recent research development for the mandibular implant-supported overdentures.
    Clinical evaluation on short-term aesthetic effect of immediate restoration of implant dentures in the maxillary anterior zone

    Abstract:Objective To evaluate the short-term aesthetic effect of immediate restoration of implant dentures in the maxillary anterior zone. Methods In Center of Implant Dentistry Affiliated Stomatological Hospital of China Medical University,select a total of 60 cases of denture implantation and reconstruction for the maxillary anterior teeth from January 2007 to January 2011,of which 30 cases with restoration half-year extension after implantation,30 cases with immediate restoration after implantation. After one-year follow-up,evaluate the aesthetic effect and satisfaction of the two groups of patients through the contour of gingival papilla,bone absorption,and satisfaction surveys. Results At a year,compared with the delayed restoration,the implant supported dentures with immediate restoration got good clinical results(P < 0.05). It showed that less bone was absorbed around the immediate restoration than around the delayed restoration(P < 0.05). The satisfaction rate of patients with immediate restoration was higher than that with the delayed restoration. Conclusion Immediate restoration is a better way to recover the contour of gingival papilla,with less bone absorption than the delayed restoration. Immediate restoration has better aesthetic effect.
    Effect of sandblasting on zirconia ceramic surface and Er∶YAG laser on dentin surface on the bonding strength of resin

    Abstract:Objective To evaluate the effect of Al2O3 sandblasting of different size particles on zirconia ceramic surface and Er∶YAG laser on dentin surface on the bonding strength of resin. Methods A total of 25 complete isolated human premolars were chosen and were cut into 50 pieces exposed dentin at Shanxi Medical University Stomatology Hospital from February 2011 to October 2011,of which 10 with 35% phosphoric acid etching,Er∶YAG laser etching processing the remaining 40. Fifty zirconia ceramic sheet were made,20 of them were not be processed,while the remaining 30(10 in each group)were sandblasted with 120,80,40 μm Al2O3 particles at 0.4 MPa for 20 s. The specimens were divided into five groups(n = 10):in group A and group B,35% phosphoric acid etching tooth blocks and Er∶YAG laser etching tooth block were bonded with no-treatment ceramic plates. In group C,D and E,the ceramic plates sandblasted with 120,80,40 μm particle size Al2O3 particles were bonded with Er∶YAG laser etching dental block. Each group was tested bond strength. Results Bond strength of group A[(8.71 ± 0.86)MPa] was lower than group B [(10.94 ± 0.93)MPa],and the difference was statistically significant(P < 0.01). Bond strength of group C,D and E [(13.49 ± 1.53),(13.97 ± 0.83),(17.11 ± 1.51)MPa]was higher than group B and the difference was statistically significant(P < 0.01);group C and D were lower than group E and the difference was statistically significant(P < 0.01);group C and D showed no significant difference(P > 0.05). Conclusion Al2O3 particles sandblasting on on zirconia ceramic surface and Er∶YAG laser on dentin surface is an effective method to increase the bond strength of zirconia ceramic and dentin.
    Clinical observation on the effect of pulse Nd∶YAG laser combined with total acid bonding system on abrasion molar

    Abstract:Objective To evaluate the clinical effect of pulse Nd∶YAG laser combined with total acid bonding system on abrasion molar during one year period. Methods Totally 40 abrasion molars were selected from 20 patients between February of 2011 and May of 2011,each of them had at least two molars or premolars according to the standard,and they were divided into two groups at random. The experimental group were treated with pulse Nd∶YAG laser combined with Scotchbond,while the control group were only treated with Scotchbond,and composite resin Z350 was selected to restore the defect. The modified USPHS criteria were used to evaluate the treatment effects at recall periods. Follow-up was conducted for one year. Results For success rate of one year,the difference between the experimental group and the control group was significant(P < 0.05). At the same time,the condition sensitivity of tooth in the experimental group was better. Conclusion The bonding strength of abrasion molar treated with Nd∶YAG laser combind with total acid bongding system can be improved.
    Assessment on the treatments for tremendous jaw cysts with fenestration and calcium sulfate implantation in adolescents

    Abstract:Objective To investigate the effectiveness of fenestration and calcium sulfate implantation in the treatment of jaw cysts,searching for a minimally invasive therapy for jaw cysts. Methods Between May 2010 and December 2011,10 cases of tremendous jaw cysts were treated,with an average age of 20.5 years. The time of fenestration ranged from 6 months to 2 years,then fill with calcium sulfate pellets after jaw cysts enucleation. Results No postoperative infection or rejection was found. The patients underwent cyst curettage and calcium sulfate implantation when jaw cysts shrinked after fenestration. At 6 months after operation,the capsule cavity boundary disappeared,there was no obvious difference in image density between the original cavity and normal bone,and the original defect area was full of new bone. Follow up the cases of jaw cysts which vanished after fenestration. Conclusion The method of fenestration and calcium sulfate implantation in the treatment of jaw cysts is ideal for bone formation.
    Research of influence of the acute stimulation on the dental pulp of injured teeth in rats

    Abstract:Objective To study the influence of the acute stimulation on the dental pulp of injured teeth in rat. Methods The first molar occlusal surface of 24 SD rats injured by grounding with turbines were randomly divided into acute stimulation group (12 rats given daily cold bath,fasting,water deprivation) and control group (n = 12,no stimulation). They were killed in batches at 5,13,20,28 days,and the gross examination,pathology section and P2X3 receptor-positive in rat dental pulp were done to explore the acute stimulation in the pathogenesis of dental caries. Results There were congestion and edema,neutrophils,lymphocytes and other inflammatory cell infiltration,and dental pulp cells degeneration and necrosis in the rat dental pulps of the acute stimulation group by HE staining pathological sections. P2X3 receptor expression increased a little at 5 and 13 days,greatly at 20 days but decreased gradually at 28 days,in acute stimulation group which showed statistical significance compared with the control group. Conclusion The acute stimulation could induce the inflammation in the pulp of the injured rat teeth,resulting in pulp pain.
    Analysis of the root canal wall stress after two kinds of root canal filling

    Abstract:Objective To analyze the relationship of two kinds of root canal filling method with the stress of root canal wall. Methods Cold gutta-percha lateral pressure method and warm gutta-percha vertical condensation method were used to fill the single root premolars. Use medical 3D image editing software to extract the gutta-percha morphology. Through a specific finite element analysis software, calculate the root canal wall stress and the stress position. And at 3,6 and 9 mm from the anatomical apix image analysis software was used to observe gutta-percha morphology and dentine cracks. Results Dentine cracks by cold gutta-percha lateral compression method were significantly more than those by warm gutta-percha vertical condensation method,and the stress of root canal wall was also higher than that of warm gutta-percha vertical condensation technique. Conclusion The root canal wall stress by cold gutta-percha lateral compression method is higher than that by warm gutta-percha vertical condensation method,and is likely to cause dentine cracks.
    Comparison of different adhesives on shear bonding strength between brackets and light-cured composite resin

    Abstract:Objective To compare different orthodontic adhesives on shear bonding strength between brackets and light-cured composite resin and to provide a reference for clinical applications. Methods Ten self-curing resin cuboid specimens were made in Department of Orthodontics,School of Stomatology,China Medical University in March,2012. The 4 surfaces of specimens were filled with light-cured composite resin. A total of 40 resin surfaces were randomly divided into groups A,B,C,D with 10 surfaces in each group. Orthodontic brackets in the four groups were bonded by Jingjin enamel adhesive,3M UniteTM chemic-cured adhesives,3M TransbondTM light-cured adhesives and light-cured composite resin,respectively. The shear bonding strength of all specimens were tested and the adhensive remnant index (ARI)was measured. Results Shear bonding strength of 4 groups was(7.763 ± 1.240)MPa(group A),(8.231 ± 0.338)MPa(group B),(8.654 ± 0.916)MPa(group C),and(8.349 ± 0.820)MPa(group D),respectively. The shear strength of four kinds of adhesive all met the clinical requirements. In group C and group A,the shear strength differences were statistically significant(P < 0.05). ARI difference did not have statistical significance in all groups(P > 0.05). Conclusion Four kinds of adhesives all meet clinical requirements while adhering brackets to polished rough surface of light-cured resin. The shear bonding strength of 3M TransbondTM light-cured adhesive is best.
