自体牙本质颗粒在上颌窦底提升术中应用的临床效果研究

Abstract: Objective    To investigate the clinical effect of autologous dentin particles applied in maxillary sinus floor elevation. Methods    A total of 22 patients(35 implants)who were scheduled for lateral window maxillary sinus floor elevation with simultaneous implant insertion at the Affiliated Stomatological Hospital of School of Stomatology,Dalian Medical University,from September 2021 to February 2023,were recruited. The patients were randomly assigned into the dentin particle group(10 patients,17 implants)and the Bio-Oss group(12 patients,18 implants)via the random number table method. During the surgery of sinus floor elevation,autologous dentin particles and Bio-Oss were respectively grafted and implants were placed concurrently. The alveolar bone height,volume and density of bone graft materials,degree of swelling,pain level,implant survival rate,incidence of complications,and patient satisfaction were compared between the two groups. Results    Compared with the immediate post-operative stage,at 6 months after surgery,the alveolar bone height in the dentin particle group and the Bio-Oss group decreased by(0.84 ± 0.13)mm and(0.73 ± 0.13)mm respectively,the volume of bone graft materials decreased by(10.48 ± 4.26)mm³ and(12.18 ± 4.59)mm³ respectively,and the density of bone graft materials(CT value)increased by(169.00 ± 65.11)HU and(140.08 ± 58.33)HU respectively. All these differences were statistically significant(P < 0.05). However,there were no statistically significant differences in the reduction of alveolar bone height,the reduction of the volume of bone graft materials or the increase of density between the two groups(P > 0.05). Also,the differences between the two groups in the level of swelling and pain were not statistically significant at 2 weeks after surgery(all P > 0.05). The incidence of maxillary sinus mucosa perforation in the dentin particle group and the Bio-Oss group was 10.0%(1/10)and 16.7%(2/12)respectively,the incidence of peri-implantitis in the two groups was 0.0%(0/17)and 11.1%(2/18)respectively,and the implant survival rate in both groups was 100%;there were no statistically significant differences(all P > 0.05). The percentage of patient satisfaction in the dentin particle group was higher than that in the Bio-Oss group,and the difference was statistically significant(Z = 2.293,P = 0.022). Conclusion    Favorable bone regeneration outcomes and clinical effectiveness can be achieved by application of autologous dentin particles in lateral window maxillary sinus floor elevation,and there is a higher level of patient satisfaction.

摘要: 目的    研究在上颌窦底提升术中应用自体牙本质颗粒的临床效果。方法    选取2021年9月至2023年2月在大连医科大学口腔医学院附属口腔医院需行侧壁开窗上颌窦底提升术同期植入种植体患者22例(植入种植体35颗)。采用随机数表法将患者分为牙本质颗粒组10例(种植体17颗)和Bio-Oss组12例(种植体18颗),在上颌窦底提升术中分别植入自体牙本质颗粒和Bio-Oss骨粉,并同期植入种植体。比较2组牙槽骨高度、骨移植材料的体积和密度、肿胀程度、疼痛程度、种植体存留率、并发症发生情况及患者满意度。结果    相较于术后即刻,术后6个月牙本质颗粒组和Bio-Oss组的牙槽骨高度分别降低(0.84 ± 0.13)、(0.73 ± 0.13)mm,骨移植材料体积分别减小(10.48 ± 4.26)、(12.18 ± 4.59)mm³,骨移植材料密度(CT值)分别增加(169.00 ± 65.11)、(140.08 ± 58.33)HU,差异均有统计学意义(均P < 0.05);2组牙槽骨高度降低量、骨移植材料体积减小量和密度增加量比较,差异均无统计学意义(均P > 0.05)。2组患者术后2周肿胀和疼痛程度各级别占比比较,差异均无统计学意义(均P > 0.05)。牙本质颗粒组和Bio-Oss组术中上颌窦底黏膜穿孔发生率分别为10.0%(1/10)、16.7%(2/12),术后1年种植体周炎发生率分别为0.0%(0/17)、11.1%(2/18),2组种植体存留率均为100%,差异均无统计学意义(均P > 0.05)。牙本质颗粒组患者满意占比高于Bio-Oss组,差异有统计学意义(Z = 2.293,P = 0.022)。结论   自体牙本质颗粒应用于侧壁开窗上颌窦底提升术中,可获得良好的骨再生效果和临床疗效,且患者满意度较高。
