Abstract: Objective To study the effect of concentrated growth factor(CGF)based sticky bone applied in horizontal bone augmentation in the maxillary anterior region. Methods Twenty patients(35 implant sites in total)requiring horizontal bone augmentation in the maxillary anterior region were selected from September 2018 to October 2020 at the Department of Implantology,West China Hospital of Stomatology,Sichuan University,and were randomly divided into 11 patients(18 implant sites)in the study group and 9 patients(17 implant sites)in the control group. The study group used sticky bone based on Bio-Oss® and CGF,and the control group used Bio-Oss® mixed with saline [conventional guided bone regeneration(GBR)technique],then compared the success rates of dental implants between the two groups at 6 months postoperatively,and analyzed width of the horizontal alveolar ridge,the thickness and density of labial bone based on cone beam CT(CBCT)measurements before operation,immediately after operation,and 6 months after operation. Results There were respectively 2 implants extracted due to poor osseointegration in each group,a total of 4 implants,at 6 months after operation. The success rate of dental implants in the study group was 88.9%,which was not statistically different when compared with the control group(88.2%)(χ2 = 0.0002,P = 0.988). Excluding the failure implant points mentioned above,the preoperative differences between the study group(16 implant sites)and the control group(15 implant sites)were not statistically significant(P > 0.05) for the width of the horizontal alveolar ridge and density of labial bone measured in imaging evaluation index. At the immediately after operation and 6 months after operation,the width of the horizontal alveolar ridge and the density of the labial bone were higher in the study group than in the control group,and the differences were statistically significant(P < 0.05). In addition,the parameters were greater than that before operation in both the study group and the control group at 6 months after operation,with statistically significant differences(P < 0.05). The thickness of the labial bone decreased by(1.2 ± 0.1)mm and(0.9 ± 0.1)mm in the study and control groups,respectively,at 6 months after operation compared with immediately after operation,with no statistically significant difference(t = 0.693,P = 0.494). Conclusion Compared with the conventional GBR technique,the application of CGF-based sticky bone in the maxillary anterior region for horizontal bone augmentation has better space maintenance ability and effect.

摘要: 目的 研究浓缩生长因子(concentrated growth factor,CGF)为基质的黏性骨块在上颌前牙区水平骨增量中应用的效果。方法 选取2018年9月至2020年10月于四川大学华西口腔医院种植科就诊的需在种植术同期行上颌前牙区水平骨增量手术患者20例(共35个种植位点),随机分为研究组11例(18个种植位点)、对照组9例(17个种植位点)。研究组使用Bio-Oss®骨粉和CGF制备的黏性骨块,对照组使用与生理盐水混合的Bio-Oss®骨粉[传统引导骨组织再生(guided bone regeneration,GBR)技术],比较2组术后6个月的种植成功率,并使用锥形束CT(cone beam,CBCT)测量分析术前、术后即刻及术后6个月的牙槽嵴水平骨宽度、唇侧骨板厚度及唇侧骨密度。结果 术后6个月,2组各有2枚种植体因骨整合不良导致松动,研究组种植成功率为88.9%,与对照组(88.2%)比较,差异无统计学意义(χ2 = 0.0002,P = 0.988)。排除可能影响测量结果的种植体松动位点行影像学评价指标的测量发现,术前研究组(16个种植位点)与对照组(15个种植位点)牙槽嵴水平骨宽度、唇侧骨密度比较,差异均无统计学意义(均P > 0.05)。而术后即刻和术后6个月,研究组牙槽嵴水平骨宽度、唇侧骨密度均大于对照组,差异均有统计学意义(均P < 0.05)。此外,研究组和对照组术后6个月的牙槽嵴水平骨宽度均大于术前,差异均有统计学意义(均P < 0.05)。相较于术后即刻,研究组和对照组术后6个月唇侧骨板厚度分别减少(1.2 ± 0.1)、(0.9 ± 0.1)mm,其差异无统计学意义(t = 0.693,P = 0.494)。结论 相较于传统GBR技术,在上颌前牙区水平骨增量手术中应用CGF为基质的黏性骨块具有更好的空间维持能力和骨增量效果。
