牙槽嵴保存联合游离龈移植改善重度牙周炎患者种植位点软硬组织条件的临床效果观察（附1例4年随访报告）

摘要： 重度牙周炎患者的治疗因其牙周支持组织破坏严重而极具挑战性，治疗方案的制定需全面考虑拔牙决策、软硬组织增量方法及修复方案等因素。文章报告1例广泛型Ⅳ期C级牙周炎伴牙列缺损的复杂病例，通过实施牙周基础治疗、拔牙同期微翻瓣牙槽嵴保存术、游离龈移植术、种植修复及定期维护，成功达到恢复健康与重建功能的诊治目标，有望为此类重度牙周炎病例的治疗提供科学的思路与系统的方案。

Abstract: The management of severe periodontitis poses significant challenges owing to the severe damage to the periodontal supportive tissues. The formulation of a therapeutic regimen necessitates comprehensive consideration of tooth extraction strategy，ways of soft and hard tissue augmentation，and prosthetic treatment design. This paper presents a complex case of generalized stage Ⅳ grade C periodontitis accompanied by tooth loss. Through the implementation of initial  periodontal therapy，micro crestal flap-alveolar ridge preservation（MCF-ARP）following extraction，free gingival graft，dental implant restoration，and periodontal maintenance，the therapeutic objectives of regaining health and reconstructing functionality are successfully attained. This case report provides potential scientific insights and a systematic approach for the treatment of such cases of severe periodontitis.
