Abstract    The proportion of early gastric cancer is low in China, which is the main reason for the poor therapeutic effect in all gastric cancer patients. It is a feasible strategy to improve the detection rate of early gastric cancer in the high-risk population that elaborate gastroscope is performed. The prognosis of early gastric cancer after radical resection is good. At present, the treatment pays more attention to both radical resection and function preservation, so as to improve the postoperative quality of life. The choice of surgical method mainly depends on the precise diagnosis of the depth of invasion, lesion extent, and lymph node metastasis of early gastric cancer before operation. Endoscopic submucosal dissection(ESD) has become the treatment of choice for early gastric cancer with a very low risk of lymph node metastasis, individualized treatment strategies should be developed for patients with non-curative resection. Various functional preservation surgery is the mainstream surgical method advocated at present. Among them, laparoscopic surgery and sentinel lymph node navigation surgery(SNNS) have been gradually recognized, and dual endoscopic surgery is one of the future development direction.

摘要： 我国早期胃癌占比低，是胃癌总体治疗效果不佳的主要原因。针对胃癌高危人群进行胃镜精查是提高我国早期胃癌检出率切实可行的策略。早期胃癌病人行根治性切除预后良好，目前治疗上更强调根治与功能保留并重，尽可能改善病人的术后生活质量。手术方式的选择主要取决于术前对早期胃癌的浸润深度、病变范围和淋巴结转移的精准判断。内镜黏膜下剥离术已成为淋巴结转移风险极低的早期胃癌的首选治疗，对非治愈性切除病人应制定个体化治疗策略。各种功能保留胃切除手术是目前外科倡导的主流术式。其中，腹腔镜手术、前哨淋巴结导航手术逐渐得到认可，双镜联合手术也是未来的发展方向。
