Abstract    Objective    To explore the status of the inpatient costs during surgical hospitalization of gastric cancer，examine the association between factors and the costs, and to provide valuable suggestions for controlling medical costs. Methods    Gastric cancer patients who had undergone surgical treatments in Peking University People’s Hospital from April 2010 to January 2014 were collected consecutively. Chi-square test and Logistic regression analysis were used to estimate the association between factors and the costs. Results    (1) The median inpatient cost was ￥83 284 （QL=58 535，QU=123 529）yuan，and the proportion of medicine fees (46.8%) was the highest among all the item fees.(2) Age，technical title of surgeon and complications were independently associated with increased cost. Conclusion    Early detection and treatment of gastric cancer and prevention of postoperative complications were effective means to control medical costs.

摘要：

目的    研究胃部肿瘤病人接受手术治疗的单次住院费用、分类的现状及其影响因素，为卫生经济政策和医疗费用管理方法的制定提供依据。方法    连续收集北京大学人民医院2010年4月至2014年1月期间因胃部肿瘤行手术治疗的住院病人，采用卡方检验和有序Logistic回归方法，分析住院总费用及其影响因素。结果    （1）住院总费用的中位费用为83 284（QL=58 535，QU=123 529）元，其中药费所占比例最高（46.8%）。（2）影响住院总费用的主要因素为年龄、术者职称以及围手术期并发症。结论    疾病早诊早治、避免出现围手术期并发症，是控制住院费用的有效途径。
