胰腺肿瘤微创手术学习曲线过程中病人安全保障对策

Abstract    At present, minimally invasive techniques have been widely used in the surgical treatment of pancreatic tumors. Still, such surgeries are difficult and risky, and how to guarantee patient safety during the longer learning curve is receiving more and more attention from pancreatic surgeons. The share of robotic surgery is overgrowing due to its unique advantages in terms of field of view and operation, driving minimally invasive pancreatic surgery (MIPS) into a new era of development. Given the large heterogeneity of existing studies, the learning curve conclusions for various MIPS procedures are not uniform. Therefore, surgeons should analyze their stage individually based on common experience and then take measures with a different focus on the current stage. In the future, each pancreatic center must conduct more high-quality clinical studies to gradually establish a standardized and objective safety assurance system for MIPS following the domestic situation.

摘要: 目前,微创技术已被广泛用于胰腺肿瘤的外科治疗,但这类手术难度大、风险高,如何在较长的学习曲线过程中保障病人安全受到了越来越多胰腺外科医师的关注。由于在视野、操作等方面具备独特优势,机器人手术的占比正快速增长,推动胰腺微创手术(MIPS)进入了新的发展时期。鉴于现有研究存在较大异质性,MIPS各种术式的学习曲线结论并不统一。因此,术者应在汲取共性经验的基础上个性化分析自身所处阶段,进而针对当前阶段采取有不同侧重的措施。未来,各胰腺中心有必要开展更多高质量临床研究以逐步建立符合国内情况的标准化、客观化的MIPS安全保障体系。
