Abstract: The quality of survey data of bariatric & metabolic surgery in Shanghai from 2019 to 2021        Group of Experts of Bariatric and Metabolic Surgery, Shanghai General Surgery Clinical Quality Control Center

Abstract    Objective    To evaluate the development trend of Shanghai's bariatric & metabolic surgery from 2019 to 2021 by analyzing the registration data. Methods    Through registration database of the Shanghai General Surgery Quality Control Center, the bariatric & metabolic surgery data which performed in 17 third-class hospitals in Shanghai from 2019 Jan to 2021 Dec were retrospectively analyzed . Results    According to the data of bariatric & metabolic surgery in 17 hospitals, annual operation case was 822 in 2019, 1031 in 2020, and 1632 in 2021, separately. Among the patients, 2335 (67.0%) were from Shanghai. There were 2504 female cases (71.9%). The average age was 32.9±12.4 years old. From 2019 to 2021, 3234 cases (92.8%) of sleeve gastrectomy (SG) and 109 cases (3.1%) of Roux-en-Y gastric bypass (RYGB) were performed. Exploratory surgeries were performed in 116 cases (3.3%), and revision surgery was performed in 26 cases (0.7%). Among the revision surgeries, there were 10 cases(38.5%) of single-anastomosis duodenoileal bypass with sleeve gastrectomy (SADI-S). The total incidence of early postoperative complications was 0.5%(17 cases), among which there were 8 cases (47.1%) of gastric leakage after SG. Compared with the data from 2016 to 2018, the total number of surgeries performed between 2016 to 2018 was 2198, with an average of 732.7 cases annually, while it was 3485 cases in sum and 1167.7 cases annually from 2019 to 2021, which had increased up to 1.59 times. SG increased from 1935 cases (88.0%,2016-2018) to 3234 cases (92.8%,2019-2021), while Roux-en-Y gastric bypass (RYGB）decreased from 148 cases (6.7%,2016-2018) to 109 cases (3.1%,2019-2021). Conclusion    Despite the impact of the COVID-19 pandemic, the total number of bariatric and metabolic surgery significantly increased in Shanghai. The surgical procedure was SG mainly, and the proportion of revision surgery and exploratory surgery had increased.

摘要： 目的    通过登记性数据分析，调查研究2019—2021年上海市开展减重代谢手术的发展变化情况。方法  根据上海市普通外科临床质量控制中心数据收集系统的登记数据，回顾性分析2019年1月至2021年12月上海17家三级医院开展减重代谢外科手术相关数据。结果    2019—2021年17家医院总手术量分别为2019年822例、2020年1031例、2021年1632例。病人中2335例（67.0%）来自上海本地，女性2504例（71.9%），年龄（32.9±12.4）岁。2019—2021年3年总手术量中，共开展胃袖状切除术（SG）3234例（92.8%），Roux-en-Y 胃旁路术（RYGB）109例（3.1%），各类探索性术式为116例（3.3%），修正手术26例（0.7%）。其中，修正手术中SG联合单吻合口十二指肠回肠旁路术（SADI-S）共10例（38.5%）。手术后早期并发症总发生率为0.5%（17例），其中SG后胃漏8例（47.1%）。与2016—2018年相比，2016—2018年手术总量共2198例，平均为732.7例/年；2019—2021年手术总量上升至3485例，平均为1161.7例/年，为2016—2018年的1.59倍。手术类型方面，SG的开展由2016—2018年1935例（88.0%）上升至2019—2021年的3234例（92.8%），RYGB开展比例由2016—2018年148例（6.7%）下降至2019—2021年的109例（3.1%）。结论    尽管有新型冠状病毒肺炎的影响，上海市减重代谢手术仍有显著增长，主流开展术式为SG，修正手术及探索性术式比例增加。
