Abstract    Incidences of groin hernias in female patients are from 3% to 6%, which are relatively rare compared with male.  Because of insufficient patients，there are no enough researches of the differences in the anatomy and structure of inguinal region for female patients. Combined with the current clinical practice and the consensus of surgeons' operation，for open hernia surgery，the round ligament of the uterus should be preserved as much as possible；the elderly patients can choose to directly cut the round ligament of the uterus during laparoscopic inguinal hernia repair surgery；for younger and childbearing patients，the peritoneotomy and re-suture method，Keyhole method should be performed to preserve the uterine round ligament as much as possible to improve the quality of life.

摘要： 女性腹股沟疝发病率为3%~6%。受制于样本量不足，对子宫圆韧带在手术中的处理方式尚存在较大争议。结合目前临床实际及外科医师操作共识，对于开放疝手术应尽量保留子宫圆韧带；高龄病人在腹腔镜腹股沟疝修补手术中可选择对子宫圆韧带直接予以横断；对于年轻、育龄期病人，可选择行腹膜切开再缝合法、Keyhole法对子宫圆韧带尽量予以保留以提高生活质量。
