三维可视化技术基础上的肝癌切除——精准与个性化的结合

Abstract    Live resection is one of the most effective therapy of liver cancer. In the actual surgical planning,there is often a contradiction between achieving adequate resection margins and retaining more liver parenchyma. By means of traditional two-dimensional image,failing to discern the variation of intrahepatic vessels and relationship between tumor and intrahepatic vessel and incapable of calculating remnant liver volume accurately makes it hard to balance such contradiction. Three-dimensional visualization model can show the variation of intrahepatic vessels and relationship between tumor and intrahepatic vessel clearly,accurately calculate the volume of liver resection,analyze the perfusion area of portal vein and the drainage area of hepatic vein,simulate diverse preoperative surgical plan,evaluate risk of postoperative liver failure quantitatively and design the customized surgical plan accurately.

摘要:

手术切除是肝癌最有效的治疗方法之一。在实际手术策略制定过程中,保证足够的肝切缘和保留更多的肝实质之间常存在矛盾。传统的二维成像无法精确判断肝内门静脉及肝静脉变异、无法观察肿瘤与血管的立体解剖关系以及无法精确计算剩余肝体积,使这一矛盾无法获得平衡。三维可视化技术可清晰地呈现肝内血管解剖及变异、肿瘤与血管的解剖关系、精确评估剩余肝体积、针对门静脉和肝静脉进行相关灌注区域及引流区域分析,并且可以在术前模拟不同的手术方案,量化评估术后肝功能衰竭风险,使肝切除更加精准且个性化。
