Abstract    By reviewing the surgical development history of colorectal cancer， the goal which colorectal surgeons are always purser is more radical surgery，much higher rate of the sphincter preservation and much better anal function. With the innovation and progress of the theory，technique and instrument，the operation of rectal cancer become more tend to be accurate and individualization. More attentions are paid to the radical cure and functional protection for low rectal cancer surgery，and it also promotes the continuous improvement of sphincter preservation operation（SPO）. Functions related factors associated with SPO include physiological，psychological， physical and time factors. All the factors will affect the functional outcome of the patient and affect the quality of life finally. Nowadays the problem for low rectal cancer with SPO is no longer can or not but is whether it should be. Fully assessing preoperatively is the way to give the individualized accuracy treatment to patients.

摘要：

手术更彻底、保肛率更高、功能更佳一直是肛肠外科医生追求的目标。随着相关的理论研究以及手术技术、手术方式、手术器械的革新和进步，直肠癌的治疗越来越趋向于精准化、个体化，特别是对于低位和极低位直肠癌，体现在肿瘤根治与功能保护并重。术式的发展演进与直肠癌术后功能密切相关，低位直肠癌保留肛门（保肛）术后功能相关因素主要包括生理因素、心理因素、物理因素和时间因素。对于结直肠外科医生而言，目前低位直肠癌保肛手术已不再是能不能保肛，而是该不该保肛的问题。术前要充分地评估病情，才能给病人个体化的精准治疗方案。
