从生物力学角度重新解读腹壁重建

摘要:

从生物力学角度重新认识腹壁的力学特性,重新解读如何进行腹壁重建,有助于选择合理的手术方式,更好的重建腹壁结构及功能,避免术后并发症的发生。从解剖学角度认识腹壁重建的核心在于修补缺损的组织,而从生物力学角度看,修补缺损的目的在于恢复腹壁力学结构的完整性及功能。临床工作中,不仅要恢复腹壁肌肉及腱性结构覆盖,也要恢复腹壁力学结构的完整性,并应用补片增加修复区域的强度。在面对巨大缺损时,应该遵循优先级原则,优先重建中央区的重要结构。此外,在应用组织结构分离技术(CST)时,须严格把握适应证,分离范围不宜过大,需要保护白线、腹直肌后鞘、脐部、腹横肌及半月线等应力结构。最后,对于术中破坏的力学结构应采用补片进行修复加强。

Abstract    Re-understanding the mechanical properties of the abdominal wall from the perspective of biomechanics, reinterpreting how to reconstruct the abdominal wall may help to choose a reasonable surgical method to better reconstruct the structure and function of the abdominal wall and avoid complications. From the anatomical point of view, the core of abdominal wall reconstruction is to repair the defective tissue, and from the biomechanical point of view, the purpose of repairing the defect is to restore the integrity and function of the abdominal wall's mechanical structure. In clinical work, it is necessary not only to restore the abdominal wall muscle and tendon structure coverage, but also to restore the integrity of the abdominal wall mechanical structure, and to apply patches to increase the strength of the repaired area. In the face of huge defects, the priority principle should be followed and the important structure of the central area should be rebuilt first. In addition, when applying CST technology, the indications must be strictly controlled, and the separation range should not be too large. It is necessary to protect the white line, posterior rectus abdominis sheath, umbilicus, transverse abdominis and meniscus and other stress structures. Finally, the mechanical structure damaged during the operation should be repaired and strengthened with a patch.
