目的    探讨胰头癌行胰腺全系膜切除的方法和策略。方法    2010年5月至2012年8月，上海交通大学医学院附属新华医院共施行胰头癌胰腺全系膜切除75例，手术标本三维空间切缘均行病理检查。结果    平均手术时间262 min，平均术中出血415 mL。17例发生术后并发症，无围手术期死亡病例。53例标本三维空间切缘病理检查达到R0切除，R0切除率为70.7%。结论    胰腺全系膜切除技术安全可行，有助于提高胰头癌R0切除率。

Abstract    Objective    To evaluate the approach and strategy of total mesopancreas excision in pancreatic head carcinoma. Methods    From May 2009 to August 2012, 75 total mesopancreas excision for pancreatic head carcinoma were performed at our institution,and surgical specimens resection margins were sent to pathologic examination in three-dimensional space. Results    The mean operation time was 262 minutes, and the mean blood loss was 415 ml. 17 cases with postoperative complications and no perioperative deaths. 53 patients were considered as R0 resection in final three-dimensional pathologic examination, and the rate of R0 resection is 70.7% .Conclusion    Total mesopancreas excision is a safe procedure in pancreatic head carcinoma, and help increasing rate of R0 resection.
