累及大静脉的复杂性腹膜后肿瘤是临床处理棘手的问题，手术中易导致大出血，手术难度大。术前应进行详细的影像学分析，评估切除瘤体和受累血管的可行性，制定详细的手术计划。采用血管外科手术技术，分离被肿瘤累及的大静脉，或切除受肿瘤累及的血管，必要时行血管重建术，以达到彻底切除病变的目的。术后妥善处理，保持受累及的血管长期通畅，避免远端脏器回流障碍，均有助于提高腹膜后肿瘤的切除率，提高病人术后生存质量。

Abstract    Surgery treatment of complex retroperitoneal tumor invading major vein is complicated because of the risk of uncontrollable bleeding. To raise the excision rate and decrease the recurrence rate, many procedures can be used. The  sensitivity and specificity of preoperative imaging examination are effective for planning surgical procedures. With vascular surgery techniques, the invaded major veins can be isolated, resected or reconstructed. Appropriate postoperative treatments, including anticoagulation therapy, are significant for long-term patency of the invaded major veins and avoidence of distal organ reflux disorder, which contribute to improve the resection rate and postoperative quality of life.
