Abstract

As the volume of metabolic and bariatric surgery continues to increase, postoperative weight regain, inadequate control of metabolic diseases, and long-term complications have become important factors affecting patients’ long-term benefits. Complex revisional metabolic and bariatric surgery is an important option for addressing these problems. However, anatomical alterations caused by previous procedures, individual differences in patients’ clinical conditions, and increased perioperative risks complicate preoperative assessment, selection of revisional procedures, surgical planning, and postoperative management, thereby placing greater demands on the accuracy and standardization of clinical decision-making. Artificial intelligence (AI) can integrate and analyze clinical data, imaging findings, endoscopic information, and follow-up data to provide multidimensional support for individualized decision-making in complex revisional surgery. Its potential applications encompass comprehensive preoperative assessment, perioperative risk prediction, selection of revisional procedures, three-dimensional assisted planning, intraoperative decision support, and postoperative follow-up management, spanning multiple stages of surgical planning and management. Although AI has an established research foundation in medical image analysis and surgical risk prediction, studies directly addressing complex revisional metabolic and bariatric surgery remain scarce, and the clinical value of relevant models requires further validation. The lack of standardized data collection, processing, and annotation; insufficient multicenter studies and clinical validation; limited model interpretability; and unclear boundaries of medical liability remain major barriers to clinical implementation. Future efforts should strengthen the development of metabolic and bariatric surgery databases, advance clinical validation, improve AI regulatory frameworks, and facilitate the effective integration of AI technologies with surgeons’ professional judgment. As a decision-support tool for surgeons, AI may improve the precision of assessment and planning, and promote the development of complex revisional metabolic and bariatric surgery toward greater precision and standardization.

Revisional bariatric procedures are increasingly common. With more primary procedures being performed to manage severe obesity and its complications, 5% to 8% of these procedures will fail, requiring revisional operation. Reasons for revisional bariatric surgery are either primary inadequate weight loss, defined as less than 25% excess body weight loss, or weight recidivism, defined as a gain of more than 10 kg based on the nadir weight; however, each procedure also has inherit specific complications that can also be indications for revision. This article reviews the history of each primary bariatric procedure, indications for revision, surgical options, and subsequent outcomes. Copyright © 2016 Elsevier Inc. All rights reserved.

To develop and assess AI algorithms to identify operative steps in laparoscopic sleeve gastrectomy (LSG).Computer vision, a form of artificial intelligence (AI), allows for quantitative analysis of video by computers for identification of objects and patterns, such as in autonomous driving.Intraoperative video from LSG from an academic institution was annotated by 2 fellowship-trained, board-certified bariatric surgeons. Videos were segmented into the following steps: 1) port placement, 2) liver retraction, 3) liver biopsy, 4) gastrocolic ligament dissection, 5) stapling of the stomach, 6) bagging specimen, and 7) final inspection of staple line. Deep neural networks were used to analyze videos. Accuracy of operative step identification by the AI was determined by comparing to surgeon annotations.Eighty-eight cases of LSG were analyzed. A random 70% sample of these clips was used to train the AI and 30% to test the AI's performance. Mean concordance correlation coefficient for human annotators was 0.862, suggesting excellent agreement. Mean (±SD) accuracy of the AI in identifying operative steps in the test set was 82% ± 4% with a maximum of 85.6%.AI can extract quantitative surgical data from video with 85.6% accuracy. This suggests operative video could be used as a quantitative data source for research in intraoperative clinical decision support, risk prediction, or outcomes studies.

Multiple patient factors may convey increased risk of 30-day morbidity and mortality after laparoscopic vertical sleeve gastrectomy (LVSG). Assessing the likelihood of short-term morbidity is useful for both the bariatric surgeon and patient. Artificial neural networks (ANN) are computational algorithms that use pattern recognition to predict outcomes, providing a potentially more accurate and dynamic model relative to traditional multiple regression. Using a comprehensive national database, this study aims to use an ANN to optimize the prediction of the composite endpoint of 30-day readmission, reoperation, reintervention, or mortality, after LVSG.A cohort of 101,721 LVSG patients was considered for analysis from the 2016 Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program national dataset. Select patient factors were chosen a priori as simple, pertinent and easily obtainable, and their association with the 30-day endpoint was assessed. Those factors with a significant association on both bivariate and multivariate nominal logistic regression analysis were incorporated into a back-propagation ANN with three nodes each assigned a training value of 0.333, with k-fold internal validation. Logistic regression and ANN models were compared using area under receiver-operating characteristic curves (AUROC).Upon bivariate analysis, factors associated with 30-day complications were older age (P = 0.03), non-white race, higher initial body mass index, severe hypertension, diabetes mellitus, non-independent functional status, and previous foregut/bariatric surgery (all P < 0.001). These factors remained significant upon nominal logistic regression analysis (n = 100,791, P < 0.001, r= 0.008, AUROC = 0.572). Upon ANN analysis, the training set (80% of patients) was more accurate than logistic regression (n = 80,633, r= 0.011, AUROC = 0.581), and it was confirmed by the validation set (n = 20,158, r= 0.012, AUROC = 0.585).This study identifies a panel of simple and easily obtainable preoperative patient factors that may portend increased morbidity after LSG. Using an ANN model, prediction of these events can be optimized relative to standard logistic regression modeling.
