糖尿病相关牙周炎的免疫代谢重编程研究进展

摘要: 血糖控制不佳可导致免疫功能紊乱,显著增加牙周炎的发病风险及严重程度。牙周炎作为糖尿病常见的口腔并发症,不仅破坏牙周支持组织导致牙齿丧失,还可能通过系统性炎症加剧糖尿病进程,形成恶性循环。研究表明,免疫代谢重编程在糖尿病相关牙周炎的发生发展中起关键作用,糖尿病高血糖环境下诱导免疫细胞代谢途径重塑,破坏其抗菌防御功能,并通过调节局部炎症微环境促进牙周组织破坏。尽管已有研究探索了部分机制,但对免疫代谢重编程在糖尿病相关牙周炎中的具体作用及潜在治疗靶点仍缺乏系统认识。文章就糖尿病相关牙周炎的免疫代谢重编程研究进展做一综述,并提出针对性干预策略,为临床治疗提供参考。

Abstract: Poor glycemic control leads to immune dysfunction,significantly increasing the risk and severity of periodontitis. As a common oral complication of diabetes,periodontitis not only destroys periodontal supporting tissues and causes tooth loss but may also exacerbate diabetes progression through systemic inflammation,forming a vicious cycle. Recent studies have shown that immunometabolic reprogramming plays a key role in the development of diabetes-related periodontitis -- the hyperglycemic environment induces metabolic pathway remodeling in immune cells,impairing their antimicrobial defense functions and promoting periodontal tissue destruction by modulating the local inflammatory microenvironment. Although some mechanisms have been explored,there remains a lack of systematic understanding of the specific role of immunometabolic reprogramming in diabetes-associated periodontitis and its potential therapeutic targets. This article reviews the research progress of immunometabolic reprogramming in diabetes-related periodontitis and proposes targeted intervention strategies,providing references for clinical treatment.
