Abstract    The lymph nodes dissection manner and range are dependent on the special characteristic of lymphatic drainage of rectum. However，lateral lymph node dissection (LLND) for low rectal cancer is still a controversial issue. Lateral lymph node dissection might decrease local recurrence of patients with rectal cancer, but with increasing risk for operation-related complications and uncertainty in improving long-term survival. Combined with preoperative imaging evaluation and sufficiently multidisciplinary discussion, plus an appropriate surgical indication, prophylactic lateral lymph node dissection still has a broad space for application.

摘要：

直肠特殊的淋巴引流特点决定了直肠癌侧方淋巴结清扫（LLND）的范围和方式。对于低位直肠癌是否应行预防性LLND目前仍存在较大分歧和争议。LLND可能降低直肠癌局部复发率，但同时也增加了相关手术并发症的发生风险，且对远期生存的改善仍不确定。结合术前影像学评估和充分的多学科讨论，把握好LLND的指征，预防性LLND仍有其广阔的应用空间。
