Abstract The lymphatic network of the stomach involves nearly every important organs and vessels in the epigastrium. As such large and confused network, the lymph node metastasis of stomach is very complicated and difficult to summarize in a simple way. Japanese Gastric Cancer Research used packet marking policy to describe stomach lymphatic system and summarized the laws of LN metastasis by the observation of positive LN distribution area in different clinical manifestations of gastric cancer. In order to ensure that researchers from surgery and pathology record data in accordance with the unified standards during the research, the Japanese Classification of Gastric Carcinoma emerged. With the deepening of the clinical research, the Japanese Classification of Gastric Carcinoma is updated continuously and always incorporated the latest research findings, in the same time it includes outstanding problems needed to be addressed to facilitate further research. So it reflects the latest knowledge of LN metastasis of gastric cancer in clinic and its amendments and the progress of treatment philosophy in gastric cancer are synchronized. After more than 50 years’ painstaking exploration, the Japanese Classification of Gastric Carcinoma has started from scratch and changed from simpleness to complexity. It is more reasonable after limit surgical exploration. Mature experience and knowledge included in the Japanese Classification of Gastric Carcinoma should be the basis of another scripture Japanese Gastric Cancer Treatment Guideline.
