Abstract Objective To evaluate the approach and strategy of total mesopancreas excision in pancreatic head carcinoma. Methods From May 2009 to August 2012, 75 total mesopancreas excision for pancreatic head carcinoma were performed at our institution,and surgical specimens resection margins were sent to pathologic examination in three-dimensional space. Results The mean operation time was 262 minutes, and the mean blood loss was 415 ml. 17 cases with postoperative complications and no perioperative deaths. 53 patients were considered as R0 resection in final three-dimensional pathologic examination, and the rate of R0 resection is 70.7% .Conclusion Total mesopancreas excision is a safe procedure in pancreatic head carcinoma, and help increasing rate of R0 resection.
