Abstract Gastrointestinal stromal tumor (GIST) is the most common mesenchymal tumor of gastrointestinal tract. Retroperitoneal GIST is a special growth pattern, which should be resected on a case-to-case basis and be consistent with the principal of surgery in GIST. Combining preoperative imatinib adjuvant therapy in selective patients, seriously evaluating the risk and benefit of surgery are crucial in bringing higher resection rate, better perseveration of organ function and lower morbidity and mortality rates of surgery.
