Abstract Perianal lesions are common in patients with Crohn's disease (CD), which consist of anal skin tags, hemorrhoids, anal fissures and ulcers, perianal fistulas and abscesses, rectovaginal fistulas, anorectal strictures and carcinoma. The surgical management of perianal Crohn's disease (PCD) should be individualized for each patient and based on the surgeon’s experience and judgment. The management of PCD should avoid surgery when active rectal inflammation is present, treat other sites of active Crohn's disease with minimal risk of incontinence, avoidance of proctectomy, achieve remission ultimately and improve patients' quality of life.
