Abstract The iatrogenic injury in choledocho-pancreatico-duodenal junction is usually difficult to discover in the course of operation because of its unique location. The iatrogenic injury can be diagnosed based on surgical procedure, postoperative symptom and imaging technique. Once iatrogenic injury is diagnosed, surgical treatment should be performed. The surgical principle includes clearing up quickly and prompt surgical drainage in order to avoid the formation of pus, thorough drainage bile following reduction of pancreatic leakage and leaky digestive juice to inhibit trypsin activation.
