Abstract The diagnosis and therapy of patients with occult breast cancer (OBC) is still a challenging for us since inability to localize the primary tumor by using conventional imaging. OBC is not detected on mammography and ultrasound based on the definition. Magnetic resonance imaging has a high sensitivity for the detection of breast cancer but with lower specificity. Although recent literature supports the use of whole breast radiation, the majority of surgeons still prefer mastectomy. The appropriate treatment of the breast after an axillary presentation of OBC continues to be a controversial issue.
