Abstract Surgery remains the main treatment for patients with soft-tissue sarcoma, and every effort should be made to attempt resection. Postoperative external-beam radiation therapy is the first choice and remains the most widely practiced local adjuvant approach because it allows for sterilization of microscopic nests of residual disease without the need to postpone surgery. Postoperative radiotherapy to conservative surgery is shown in a randomized trial to enhance local control compared conservative surgery alone. Intensity-modulated radiation therapy in soft-tissue sarcoma provides excellent local control with a beneficiary effect in sparing normal tissues, which might be the main model of radiotherapy.
