Abstract    Lateral lymph node metastasis and surrounding tissue infiltration are main factors in the process of proliferation and development of lower rectal cancers, which also playing a critical role in the recurrence of lower rectal cancer after Miles surgery. SAR based on characters of proliferation、development、metastasis and  recurrence of lower rectal cancer,  which  facilitated  the possibility of an exact, sharp, and directly visualized removal of all structures critical to local recurrences of rectal cancer, and achieved the TME and optimal lateral lymph node dissection. SAR is an effective means in surgical treatment of lower rectal cancer, which effective reduces local recurrence and improve survival rates.
