Abstract Surgery should be considered as a rational therapy for gastric cancer with liver metastases (GCLM), some of whom can survive long term after surgery. Neoadjuvant chemotherapy and transarterial therapy have been demonstrated efficient to downstage tumor and increase the curative R0 resection rate. However, the optimal indications and effective results of surgery and neoadjuvant therapy of GCLM remains a matter for debate. Therefore, further studies from multicenter, prospective, randomized trials are necessary to determine which patients will benefit from surgery of GCLM.
