Abstract    Iatrogenic bile duct injury is one of severe complications of abdominal surgery with an increasing morbidity, especially after cholecystectomy. Less attention was paid to several special issues of the etiology of bile duct injury which may toughen the diagnosis. Moreover, a consensus on the classification of bile duct injury is still reaching yet. To iatrogenic bile duct injury, the crucial alternative to prevention should be an intimate knowledge of the regional anatomy and there will be a better prognosis if treatment is given and supervised by an experienced specialist of biliary surgery. More attention should be paid to the associated vascular injury, especially to associated injury of the right hepatic artery.
