Abstract With the evolution in the management of rectal cancer, especially for the implementation of total mesorectal excision and neoadjuvant therapy, local recurrence rate has decreased significantly. However, the optimal treatment of locally recurrent rectal cancer (LRRC) remains a difficult and controversial issue because patients have received the neoadjuvant or adjuvant therapy during the primary rectal cancer treatment. Radical resection of the recurrence offers the best chance of cure. But it should be considered only in carefully selected patients. Therefore, multimodality therapy should be the best chance under the circumstances and the optimal options of multimodality therapy have become the point now.
