Abstract Postoperative local recurrence of rectal cancer is a major clinical concern. How to make reasonable and individual clinical decision is the decisive step to improve patients’ outcome. The preoperative assessment is important, which could help doctor make clear about the clinical stage and other useful information of recurrent tumor. Radical resection is the most critical prognostic factor. The preoperative radiotherapy and chemoradiotherapy are also effective for tumor control. In addition, a part of patients may get survival benefit from clinical multidisciplinary team discussion.
