Abstract The primary gallbladder carcinoma is the most common malignant tumors in the biliary system, which has poor prognosis with 5%~13% actuarial survival at 5 years. Resection is the certain means for patients with gallbladder carcinoma. The decision about the type of resection to be performed should be mostly based on patients’ status, tumor characteristics and Nevin stage. The early gallbladder carcinoma with Nevin I stage only need simple cholecystectomy. A more aggressive surgical approach, including resection of the liver, gallbladder bed and regional lymph nodes is advisable for patients with Nevin II to V tumors. Chemotherapy and radiation treatment are valid to prolong survival.
