Abstract Anticoagulation therapy does not treat deep venous thrombosis (DVT) by dissolution of thrombus but instead prevents the propagation of the existing acute DVT. A significant number of patients, particularly those with iliofemoral DVT, will develop the post-thrombotic syndrome (PTS), despite receiving anticoagulation therapy. Yet a growing body of evidence suggests that a treatment strategy that includes early thrombus removal plus optimal anticoagulation may help prevent PTS.
