Abstract Objective To explore whether the sac of direct inguinal hernia can extend into scrotum and the impact factors. Methods The clinical data of 417 cases of direct inguinal hernia admitted bertween September 2003 and October 2008 at Treatment and Training Center of Hernia and Abdominal Wall Surgery,West China Hospital of Sichuan University were analyzed retrospectively. Results Sacs extending into scrotum occurred in 7 cases (1.7%). Compared with cases whose sacs were not extended into scrotum, those cases had a longger duration and bigger hernia rings or sacs. Those cases were usually with some factors, such as chronic cough, that may make internal abdominal press higher. Conclusion The sac of direct inguinal hernia may extend into scrotum and the impact factors are high ages, long duration, large hernia masses and chronic bronchitis making internal abdominal press higher.
