Abstract Objective To evaluate the outcome of spleen-preserving distal pancreatectomy and explore its feasibility. Methods The clinical data of 22 cases of spleen-preserving distal pancreatectomy from April 1993 to July 2009 in the First Affiliated Hospital of China Medical University were analyzed retrospectively. Results Among them, 8 cases of non-functioning islet cell tumor, 8 cases of cystadenoma, 5 cases of insulinoma and 1 case of pseudocyst were included. There were no postoperative deaths, pancreatic fistula occurred in 5 patients, and was cured within 2~3 weeks by non-operative treatment. Conclusion Spleen-preserving distal pancreastectomy is the preferred procedure for certain benign lesions of pancreatic body or tail that are not candidates for enucleation. This procedure has good result at short-term after the operation.
