Corresponding author: MA Yong, E-mail: doctormy@yahoo.cn Abstract This article mainly discusses several issues in clinical research in surgery that deserve attention currently, and points out the principal difficulties in elevating the level of these clinical researches, such as control establishment, randomness and data collection. According to the tendencies in the quality of highly cited surgical research over the past 20 years, it shows that the surgical scholars are trying to overcome the difficulties mentioned above, so as to improve their research work. From our surgical researchers' own point of view, we need to pay enough attention to several aspects, including innovation, subjects, and characteristic and so on, and seek truth from facts, then we could keep promoting the level of clinical research under the existing research platform in China.
