The value of mesentericoportal vein resection (VR) associated with pancreaticoduodenectomy (PD) for pancreatic-head adenocarcinoma still remains controversial. Affirmatively, venous resection can be safely performed. Pre-and Intraoperative assessment is needed to define the relationship of a pancreatic head neoplasm to adjacent vascular structures. Surgeons who perform PD should have experience of vascular surgery.
