The malignant lesions in the pancreatic head were more common than the benign lesions, so the delay of diagnosis would leave the patients in a serious situation of losing the possibility of cure. For the reason that the incidence of peritoneal carcinomatosis of tumor cells results from pre-operative traumatic biopsy, the intra-operative puncture biopsy would hold advantages. Even if no accurate diagnosis achieved, on the premise of extensively communicating with patients and families, actively removing the lesions through different procedures should be a trend of the managements for this kind of diseases in the future. Although the masses in the pancreatic head originate from various etiology, in the clinical practice we should employ the diagnostic and management modalities for the common diseases with comprehensive judgment and correct disposal.
