Abstract

Organ preservation and damage control in surgical treatment of pancreatic head benign and low malignant lesions WANG Chun-you. Pancreas Surgery , Union Hospital, Tongji Medical College, Huazhong University of Science and Technology, Wuhan 430022, China Abstract The pathological types of space occupying lesions of pancreatic head are very complicated that including high malignant tumor, low malignant tumor, benign tumor and inflammatory mass. The main surgical procedure for space occupying lesions of pancreatic head is Whipple procedure. However, whether the Whipple procedure is proper for low malignant and benign lesions of pancreatic head is still a controversial issue. In this paper, we introduce the organ-preserving procedures, e.g. pancreatic enucleation, duodenum-preserving pancreatic head resection and pylorus-preserving pancreaticoduodenectomy, for low malignant and benign lesions of pancreatic head, and promote the principles of damage control and organ preservation, which will be beneficial to the decrease of postoperative mortality and morbidity, sustainment of patients,physical function, improvement of quality of life, control of hospital charge and decrease of medical tangle.
