根治性手术联合淋巴结清扫是胆囊癌唯一可能被治愈的方式，手术方案根据胆囊癌分期而定，但手术切除术范围、淋巴结清扫范围等方面仍存在不少争议。进展期胆囊癌行扩大根治术并不能使病人获得令人满意的生存获益，需要谨慎对待。以根治性手术为基础的综合治疗才有可能最大程度地改善胆囊癌病人的预后。

Abstract    Surgical resection associated with lymphadenectomy is the only chance to cure gallbladder cancer, the specific surgical plan is based on the staging, but several aspects of surgical management are controversial, including operative extent, and lymphadenectomy extent. Extended radical resection for advanced gallbladder cancer does not provide satisfactory survival benefits for patients and should be treated with caution. The comprehensive treatment based on radical resection is likely to improve the prognosis of gallbladder cancer patients to the greatest extent.
