减重与代谢外科手术新术式机制研究进展#br#

摘要: 肥胖是严重的全球性健康问题,带来巨大的社会经济负担。肥胖同时也是众多慢性疾病的危险因素。减重与代谢手术(MBS)是治疗肥胖最有效的方法。全球范围内最新开展的几种MBS主要包括胃袖状切除术联合单吻合口十二指肠转位术、单吻合口胃旁路术,十二指肠-空肠旁路术,空肠-空肠旁路术以及内镜胃袖状成形术。手术主要通过限制摄入、吸收不良或二者相结合的方式达到减重目的,并且可以影响食欲,改变摄食习惯,通过改变胃肠道激素、胆汁酸分泌水平影响代谢,术后肠道菌群改变可能是手术的额外机制。胃袖状切除术联合单吻合口十二指肠转位术和单吻合口胃旁路术更值得推荐。

Abstract    Obesity is a serious global health problem with a huge socio-economic burden. Obesity is also a risk factor for many chronic diseases. Bariatric surgery is the most effective way to treat obesity. The most recent mechanisms of action in several bariatric surgeries performed worldwide, including single anastomosis duodenal-ileal bypass with sleeve, one anastomosis gastric bypass, duodenojejunal bypass, jejune-jejunal bypass, and endoscopic sleeve gastroplasty. Surgery achieves weight loss mainly through restriction of intake, malabsorption or a combination of both, and can affect appetite, alter ingestive habits, influence metabolism by altering levels of gastrointestinal hormones, bile acid secretion, and postoperative intestinal flora changes may be additional mechanisms for surgery. In the author’s opinion, single anastomosis duodenal-ileal bypass with sleeve gastrectomy and one anastomosis gastric bypass are more recommendable.
