应用胃悬吊技术行单孔腹腔镜胃袖状切除术围手术期疗效对比研究#br#

摘要:  目的    探讨单孔腹腔镜胃袖状切除术(SILS)中应用胃悬吊技术的围手术期疗效。方法    回顾性分析2023年2~4月复旦大学附属华东医院完成的36例SILS病人临床资料。根据术中是否行胃悬吊将其分为胃悬吊组(n=20)与对照组(n=16)。对比分析两组病人的手术时间、术中出血量、引流管放置率、术后C-反应蛋白(CRP)、住院时间、并发症发生率等临床数据。结果    两组病人均顺利完成手术,无中转开放手术。胃悬吊组的手术时间明显短于对照组[(98.2±18.6)min vs. (131.6±20.2)min,P<0.05],术中出血量明显少于对照组[(25.2±8.1)mL vs. (33.5±12.0)mL,P<0.05],引流管放置率明显低于对照组(0 vs. 0.1,P<0.05),;两组间术后第24h CRP水平、住院时间、术后并发症发生率、再入院率等差异无统计学意义(P>0.05)。结论    通过采用胃悬吊技术能够顺利开展SILS,术中显露满意,可缩短手术时间。

Abstract    Objective    To investigate the perioperative effect of gastric suspension technique in single incision laparoscopic sleeve gastrectomy (SILS). Methods    The clinical data of 36 cases of SILS patients in Huadong Hospital Affiliated with Fudan University from February to April 2023 were analyzed retrospectively. The patients were divided into the gastric suspension group (n=20) and control group (n=16) according to whether the gastric suspension was performed during the operation. The clinical data of operation time, intraoperative blood loss, rate of drainage tube placement, postoperative C-reactive protein (CRP), length of stay, incidence of complications, and so on were compared and analyzed between the two groups. Results    Both groups of patients successfully received SILS successfully with no conversion to open surgery. The operative time of the gastric suspension group was significantly shorter than that of the control group [(98.2±18.6) min vs. (131.6±20.2) min, P<0.05], and intraoperative blood loss was significantly less than that of the control group [(25.2±8.1) mL vs. (33.5±12.0) mL, P<0.05]. The rate of drainage tube placement was significantly lower than that of the control group (0 vs. 0.1, P<0.05). There were no significant differences in CRP level 24h post-operation, length of stay, incidence of postoperative complications and readmission rate after surgery between the two groups (P>0.05). Conclusion    By means of the gastric suspension technique, SILS can be successfully carried out and provide a better surgical view, which can shorten the operation time.
