超远程手术机器人辅助直肠癌根治术临床初步研究

摘要: 目的    探讨国产手术机器人系统联合5G通信技术行超远程机器人辅助直肠癌根治术的安全性及可行性。方法    2024年1月4日,中国人民解放军总医院普通外科医学部在北京市与三亚市之间(距离约3000 km)应用国产手术机器人系统完成国内首例超远程机器人辅助直肠癌根治术。结果    病人男性,66岁,因“间断腹痛、腹胀1个月余”入院,经肠镜检查及病理诊断为直肠癌,有明确手术指征,无明确手术禁忌证。2024年1月4日,病人于全麻下行超远程机器人辅助直肠癌根治术,手术过程顺利流畅,无延迟现象,无手术并发症。术中出血量20 mL,手术时间90 min。术后第3天排气,第4天进食流食,术后第7天出院。术后病理检查结果提示淋巴结清扫彻底(1/15),直肠系膜切除完整,环周切缘及上下切缘均未见癌。结论    初步证明5G通信技术支持下超远程机器人辅助直肠癌手术安全可行,值得进一步探讨和实践。

Abstract    Objective    To explore the safety and feasibility of ultra-remote robot-assisted radical resection of rectal cancer by domestic surgical robot system combined with 5G communication technology. Methods    On January 4,2024,Department of General Surgery of Chinese PLA General Hospital completed a case of ultra-remote robot-assisted radical resection of rectal cancer using a domestic surgical robot system between Beijing and Sanya,Hainan(about 3000Km). Results    A 66-year-old male patient was admitted to the hospital for having suffered intermittent abdominal pain and abdominal distension for more than one month.He was diagnosed with rectal cancer by colonoscopy and pathological examination,with clear surgical indications and no clear surgical contraindications.On January 4,2024,the patient underwent ultra-remote robot-assisted radical resection of rectal cancer under general anesthesia.The operation was successful and smooth with no delay and no surgical complications.The intraoperative blood loss was 20 mL,and the operation time was 90 min.The patient passed flatus on the 3rd day,ate liquid food on the 4th day,and was discharged on the 7th day after operation.Postoperative pathology showed complete lymph node dissection(1/15),complete mesorectal excision,and no cancer at the circumferential margin and the upper and lower margins. Conclusion    Ultra-remote robot-assisted rectal cancer surgery supported by 5G communication technology is safe and feasible,which is preliminarily demonstrated and worthy of further discussion and practice.
