目的    探讨复发性腹膜后脂肪肉瘤的临床特点、复发因素及诊治方法。方法    回顾性分析哈尔滨医科大学附属第二医院2002—2012年收治的37例腹膜后脂肪肉瘤病人13例22次复发的临床资料。结果    复发病例较初发者更容易引发临床症状和侵袭周围组织或脏器，手术完全切除率低；随着复发次数的增加，复发间期逐渐缩短，恶性度增加，生存期明显缩短。结论    腹膜后间隙的特殊性和肿瘤形态结构的多样性是导致腹膜后脂肪肉瘤不易被彻底切除和术后复发的重要因素，强调首次完全完整和彻底的切除是防止复发、改善预后的关键。

Diagnosis and treatment of recurrent retroperitoneal liposarcoma: An analysis of 37 cases        WU De-quan. Department of General Surgery, the 2nd Affiliated Hospital of Harbin Medical University, Harbin 150086, China
Abstract    Objective    To investigate the clinical characteristics and treatments and key factors of recurrent retroperitoneal liposarcoma（RPLS）. Methods    The clinical data of 37 patients with initial and recurrent RPLS admitted between 2002 and 2012 in the 2nd Affiliated Hospital of Harbin Medical University were analyzed retrospectively．Among 37 patients, there were 13 patients with 22 case-times of postoperative recurrence totally. Results    Compared with initial patients, recurrent patients had the following characteristics：the worse biological behaviors，more likely to lead to clinical symptom, more obvious physical examination positive signs, stronger tumor invasive power, less complete resection and longer time of hospital stays. The interval without tumor burden turned shorter after multiple recurrences，and the survival rate become lower. Conclusion    The characteristics of retroperitoneal space and variousness of tumor’s morphology are important events for recurrence of retroperitoneal liposarcoma. It is the key factor to prevent recurrence and improve prognosis by critical complete and intact resection during the initial process.
