75%的十二指肠憩室是十二指肠乳头旁憩室，由憩室引起的临床症状经内科治疗无效或合并有胰胆疾病时需要手术治疗。对十二指肠乳头旁憩室的手术要严格掌握适应证。手术方式的选择取决于临床表现及憩室的部位，胆汁或食物转流手术比憩室切除更安全有效。

Abstract    About 75% of duodenal diverticulas are located near papilla, which are also named as juxtapapillary duodenal diverticula (JPDD). For patients with symptoms complicated with pancreatobiliary disorder or not response to medical treatment，it is necessary to perform surgical intervention. Operative indication of JPDD should be chosen strictly. Surgical methods mainly depend on clinical manifestation and location of JPDD. Considering high risk for JPDD resection，it is more safe and effective to perform bile and/or food by-pass operations sometimes.
