所谓“特殊类型乳腺癌”一般涵括了以组织病理定义的特殊类型乳腺癌，以及临床约定俗成但并无准确定义分类的部分乳腺癌。后者由于发病率低，缺少有针对性的前瞻、随机对照研究。因此，包括美国国立综合癌症网络（NCCN）在内的临床指南和诊治规范并未单独列出其诊治流程，临床往往出现认识上的误区。笔者认为，无论组织病理定义，还是临床约定俗成，都是以达到对疾病更为深刻的生物学认识为基本目的。而所谓特殊类型的乳腺癌，其中绝大多数的个体化诊治原则与非特殊型乳腺癌并无违背。在此基础上参考其临床特点、影像学特征以及与预后评价相关的特殊性，综合分析生物学行为和疾病分期，以达到更为科学的个体化治疗原则。

Abstract    The special type breast cancer have no clearly definition. Commonly, this term was defined from double sides: pathology and clinic. Because of the lower incidence and lack of large random and prospective data, the diagnosis and treatment were rarely mentioned in various guidelines, and some study conclusions maintained controversial. Regardless the definition of special type breast cancer, it is important to reveal the biological nature of a special type breast cancer and make individual treatment plan based on widely accepted international guidelines and the Ministry of Health standard. Generally, clinical practice of the special type breast cancer can learn from invasive ductal carcinoma of no special type, and meanwhile perform the individual treatment according to stage, clinical specialty, and biological characters.
