病灶切除和消化道重建是肝门部胆管癌手术治疗的两个关键环节。进行肿瘤根治性手术时，注重肿瘤彻底切除的同时需兼顾胆肠吻合安全。肿瘤姑息性手术则应侧重于胆肠吻合的确实，以尽可能改善病人的生活质量为主要目标。

Abstract    Tumor resection and biliary-enteric anastomosis are the two key steps of surgical treatment of hilar cholangiocarcinoma. The security of the anastomosis should be taken into account when radical resection of hilar cholangiocarcinoma is performed. However, it is more important that the anastomosis should be performed reliably in palliative surgical treatment of tumor, because the goal of surgery is to improve patients’ satisfaction and quality of life as much as possible.
